Worker falls to his death from lift; OSHA issues fine

A company faces OSHA fines in connection with the death of a cameraman at a motocross event.
Videographer Stuart Keene fell about 20 feet on June 25, 2010, while setting up a camera for an event at the Thunder Valley Motocross race course near Morrison, CO.
OSHA has issued $91,000 in fines to Lucas Oil Production Studios of California for:
- a willful citation for deliberate removal of guardrails from scissor lifts exposing employees to falls of up to 27 feet
- a serious citation for failure to provide training to employees on proper operating procedures for all-terrain scissor lifts
- a serious citation for exposing employees to crushing and amputation hazards by not having established a lockout program for performing modifications to the lifts, and
- a less-than-serious citation for failing to report the death.
After meeting with OSHA and getting a copy of the investigation findings, Keene’s brother, Christopher Keene, told a Denver TV station, “All of this should have been prevented, could have been prevented.”
