1 worker dead, 4 injured after tire explodes

OSHA is trying to figure out what went wrong at a factory where a tire exploded, killing one worker and injuring four others.
Several employees at Harsco Metals in Ghent, KY, were trying to fuse a rim to a large tire on a piece of heavy equipment, when the tire exploded, killing 48-year-old Robin Kelley.
One other worker was flown to the University of Cincinnati hospital in serious condition. Three other employees were taken to a local hospital with non-life threatening injuries.
